Pap test (standard and LBC)

The PAP test is a cytological examination of the cervix used to detect precancerous and early stages of cervical cancer, as well as inflammatory and reactive changes. It represents a fundamental part of preventive gynecological care.

In gynecological practice, the standard PAP test and the liquid-based PAP test (LBC – Liquid Based Cytology) are used.

Standard PAP test

The standard PAP test is performed by collecting cells from the surface of the cervix, which are directly smeared onto a glass slide and then cytologically analyzed.

Advantages of the standard PAP test:
widely available, a method proven over many years, reliable in detecting cytological abnormalities.

Limitations:
possible presence of inadequate samples due to blood, mucus, or inflammation; lower sensitivity compared to the LBC method.

Liquid-based PAP test (LBC)

The LBC PAP test involves collecting cervical cells that are stored in a liquid medium, and then processed in the laboratory under standardized conditions.

Advantages of the LBC method:
higher sample quality, fewer inadequate findings, greater sensitivity and specificity, the possibility of additional analyses from the same sample (HPV testing).

The LBC method is now recommended in modern guidelines for cervical cancer screening.

When is a PAP test done

Preventively, as a rule once a year or according to the gynecologist’s recommendation, after the onset of sexual activity, when monitoring previous abnormal findings, as part of a regular gynecological examination.

How to prepare for a PAP test

It is recommended to come outside of menstrual bleeding, avoid sexual intercourse, vaginal medications, and vaginal douching 24–48 hours before sample collection, and bring previous cytological findings if available.

Is the PAP test painful?

The PAP test is not painful, takes a short time, and may be mildly uncomfortable. The sample is taken during a gynecological examination using a speculum.

PAP test and HPV testing

The PAP test and HPV testing are not the same, but they complement each other.

In certain age groups and clinical situations, combined screening (PAP + HPV test) is recommended, especially with the LBC method.
